Contribution and recognition forms are now combined and revised

The Global Contribution and Recognition Transfer forms have been revised and combined into one form as part of The Rotary Foundation's ongoing efforts to provide more accurate processing and faster acknowledgement to our donors.

The new version is now available as one double-sided document, and The Rotary Foundation Contribution Form (PDF) includes a recurring gift option for donors who wish to give monthly or quarterly through Rotary Direct.

Another new feature is a Quick Response (QR) code, which is located at the top of the form and allows users with mobile devices to scan and land directly onto Rotary's online giving page. Rotary’s secure online contribution system also supports both one-time and recurring gifts.
